The first step in getting started with pediatric occupational therapy is to complete our client intake forms. These forms provide important information about your child’s development, daily routines, strengths, and areas of concern. This allows our team to begin building a clear understanding of your child and how we can best support your family.

We encourage families to take their time when completing the forms and to share as much detail as possible. This information helps us prepare for next steps and ensures that your child’s evaluation and care are personalized from the very beginning.

Once your intake forms have been submitted, our team will carefully review the information provided and begin coordinating the next steps.

A member of the Maple Tree Pediatric Therapy team will reach out to you within three business days to follow up. 

We will also begin coordinating any necessary documentation, including obtaining a referral from your child’s provider if required and verifying insurance benefits.

Following our initial conversation, we will schedule your child’s occupational therapy evaluation at a time that works for your family.

We understand the importance of timely support when concerns arise. Depending on current scheduling and therapist availability, we strive to schedule evaluations as quickly as possible while ensuring each child receives thoughtful and individualized care.

After your child’s evaluation is completed, we will review the results with you and develop a personalized plan of care based on your child’s needs and goals. If ongoing therapy is recommended, sessions will be scheduled in collaboration with your family.
