How to Get Started with Maple Tree Pediatric Occupational Therapy Services
There are three pieces to getting started with OT Services with Maple Tree Pediatric Therapy in Ashe County. The first is to complete the Client Intake Forms (below). These forms can be completed on a computer or phone and take about 20 minutes. The questions will go over goals you may have for you child, their interests, medical history, and insurance information.

Pediatrician Referral
The second step is to obtain a doctor’s referral from the pediatrician and review insurance coverage/benefits. Our team at Maple Tree Pediatric Therapy will take care of both of these pieces as soon as we have your information!

Schedule an Initial Evaluation
The third step is to schedule and complete a comprehensive Pediatric Occupational Evaluation. Typically, this includes a 90-minute session in the clinic with standardized materials and components. The evaluation piece can also include a caregiver interview, an observation time at home or school, a completed survey from a caregiver, and collaboration with other therapists or providers who may currently or previously have worked with the client.
Post Evaluation Steps
Once the evaluation is complete, families will receive a written copy and meet to discuss the results and determine the next steps and goals. If therapy is recommended, weekly sessions will be scheduled and set up.
